Koh Samui repatriation planning guide
Use this Koh Samui guide to name the actual island, distinguish USM departure from a mainland plan, and identify any pier, sea and road handovers. Every proposed transport stage needs its own assessment and confirmation.

Begin a Koh Samui enquiry with the person’s present island and care location. State clearly if the person is on Samui, on another nearby island or already on the Surat Thani mainland. A hotel reservation or return-flight booking may describe the wider holiday without identifying where assistance would begin. Give the hospital, facility or accommodation contact who can confirm the current position.
Bangkok Airways identifies Samui International Airport as being on Koh Samui in Surat Thani, and its airport and cargo information uses the code USM. The island airport and a mainland departure are different starting arrangements. If a proposal mentions “Surat Thani”, ask whether it means the province, a mainland location or a specific airport, and request the full name and code.
Treat a sea stage as a separate decision
Tourism Authority of Thailand mapping distinguishes Samui’s Nathon and Lipa Noi piers from Don Sak on the mainland. Those geographic names help show why an island-to-mainland plan needs more than a road pickup instruction. If any sea movement is proposed, name the departure and arrival pier and ask the responsible professionals to assess whether that stage is suitable and available for the case.
The family should know who handles the movement to the pier, the sea stage and collection at the other end. Ask how responsibility passes between providers and what would happen if the planned crossing could not proceed. A public ferry route is not evidence of medical support, authorized carriage of human remains or an agreed transfer arrangement. Do not book a crossing to force an otherwise unconfirmed itinerary.
Plan an airport departure independently of ferry options
If departure from USM is being considered, identify the intended destination, operating airline and any connection. Ask which professionals will check acceptance and the airport handover before the local collection is fixed. A flight from the island may avoid a proposed mainland stage, but that fact alone does not make it the appropriate or available arrangement for a particular patient or deceased case.
For a connection, give every flight and airport code. If the plan passes through Bangkok, check whether the onward flight uses the same airport. Ask whether waiting, a change of team or additional ground transportation is included. Ask clinicians to assess each change of transport
For a patient, provide the treating team’s contact and the proposed receiving setting. Ask who reviews the complete movement, including any road, pier, sea and airport stages actually being considered. The review should address the patient’s current needs and the care required during handovers. Relatives should describe practical circumstances accurately without trying to select clinical equipment or decide transport suitability themselves.
For commercial flights, the airline may request medical evidence before clearance. Ask who deals with the operating carrier and how an approval relates to the agreed itinerary. If a medical escort is proposed, establish where the escort joins and finishes. Do not assume that an airport assistance request, boat crew or accompanying relative provides the clinical responsibilities described in an escort proposal.
Establish custody and release before moving a deceased person
After a death, identify the current custodian and the authority responsible for release. Explain if the death occurred on another island and the person has since been moved to Samui or elsewhere. Keep those locations distinct so the appointed professionals can identify relevant records and contacts. Do not interpret a transfer between facilities as confirmation that international transport is ready.
The nominated family representative should discuss the intended arrangement with the funeral professional and the relevant consular office. Identify the endpoint of an island quotation
A written quotation should state where responsibility begins and whether it ends at a Samui facility, pier, airport or destination beyond the island. If the proposal includes mainland transport or an onward flight, ask for those stages to be identified explicitly. Check who authorizes changes when a flight, access arrangement or release decision changes. A single phrase such as “Samui transfer” does not define those obligations.
Ask the insurer which appointment and expenses it has approved, where applicable. Keep a list of confirmed contacts alongside outstanding decisions rather than treating every discussed option as a booking. The enquiry channel can receive an outline of the case. Local providers, island collection coverage, sea transport, airport handling and any specific route must all be confirmed individually before the family relies on them.
